Overview

We are looking for a kind and caring Community Carer to join our team. The ideal candidate will offer key support to people in their own homes or care settings, supporting independence and well-being. Responsibilities

• Help clients with daily chores including personal care, meal prep and medication reminders
• Assist clients to preserve independence in their homes or care surroundings
• Drive clients to appointments, social activities or errands in business vehicles or personal transportation if appropriate
• Develop and implement individualised care plans to meet each individual’s needs
• Monitor and report on clients’ health state and wellness, ensuring that concerns are reported properly to necessary professionals.
• Keep accurate records of care provided and any changes in the client’s condition
• Improve the quality of life of customers through promoting social engagement and emotional support
• Follow health and safety procedures, including infection control methods

Abilities

• Experience of working in a care home or providing home care is ideal but not essential; training will be given if appropriate
• Ability to establish and implement individualised care plans efficiently.
• Excellent communication abilities (verbal and written) in English to comfortably liaise with clients and team members
• Competent in the use of IT systems for record keeping and reporting
• Excellent interpersonal skills to demonstrate empathy, patience and understanding of clients’ needs
• Excellent organisational skills to successfully co-ordinate schedules and prioritise activities effectively
