Position summary
We are looking for a caring and enthusiastic Children’s Community Health Care Assistant to join our Children’s Home Care Team.

This is a fulfilling profession where you will give high-quality, tailored care to children and young people with a variety of frequently complicated health problems, some of whom may be technology dependent. You will be working on a one-to-one basis, supporting children in their own homes, schools, and other community settings around Somerset.
We cover a vast geographical area, thus you will be expected to travel to the child’s location, which may involve journeys of over 1 hour (mileage allowance paid). You will be part of a 365-day 24-hour service, you will need to be flexible.
You will undergo extensive training to meet the individual requirements of each child and then work with experienced team members for a period of time to gain confidence and competence in the delivery of care.
Key duties, tasks & skills required
- Deliver high-quality one-to-one care to children and young people with complex and unexpected health needs in the home, school, or community setting.
- Fully trained to support youngsters who might be dependent on medical technology such as ventilators, feeding systems, and suction devices.
- Provide care in line with individual care plans, rules, and procedures at all times, ensuring the safety and well-being of the child.
- Work closely with families, carers and the interdisciplinary team, communicating clearly, sensitively and professionally.
- Assist with personal care, mobility, eating, and medication administration as required, emphasising independence as far as feasible.
- Recognise and respond to changes in a child’s condition, and escalate concerns as necessary.
- Maintain clinical records that are accurate and up to date in conformity with Trust policy.
- Work in a flexible way to suit the demands of the service, including days, waking nights, weekends, bank holidays, and emergency cover.
- Travel throughout Somerset to suit the needs of the service.
DBS (Disclosure & Barring Service) Check
This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975, and as such, it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (previously CRB) to check for any previous criminal convictions.
