As a Band 6 Occupational Therapist within the Harlow Integrated Community Care Team (ICCT), you will play a key role in supporting the delivery of proactive, neighbourhood-based care that enables people to remain independent and well within their own homes and communities. Working as part of a multidisciplinary team, you will provide specialist occupational therapy assessment and intervention for individuals with complex physical health needs, focusing on preventing hospital admissions, facilitating timely discharge, and reducing health inequalities. You will work closely with local Primary Care Networks, community services, social care, voluntary sector partners, and neighbourhood teams to deliver coordinated, person-centred care plans that promote independence, wellbeing, and self-management. Qualifications

Essential

  • Degree or equivalent in Occupational therapy
  • Relevant training appropriate to role

Experience

Essential

  • Broad range of experience across a sample of clinical areas
  • Evidence of wide range of clinical and non-clinical experience

Knowledge

Essential

  • Knowledge of NHS policies and Neighbourhood and proactive care, preventing admissions and enabling people to remain independent at home.
  • Experience of rehabilitation and functional assessment
  • Experience of working with adults with physical health needs

  • Access to vehicle for work
