Primary responsibilities of the position

  • To promote and oversee the physical and mental well-being of service users while serving as a crucial and highly esteemed part of the Multi-Disciplinary Team under the guidance and supervision of community mental health professionals. delivering holistic, person-centred treatment that empowers clients to pursue their rehabilitation objectives.
  • In order to support service users’ recovery journeys, the Community Support Worker will assist each service user in locating, obtaining, and participating in local community support, as well as in creating an individual support network and independent living skills.
  • In order to encourage recovery or stop deterioration, the position holder will carry out our Carers Assessments and collaborate with service users and carers who have been determined to have eligible social care needs.
  • They will be required to follow Trust policies and procedures, work with little supervision, and use socially inclusive and value-based practices. While they won’t be in charge of safeguarding investigations, Community Support Workers will be in charge of identifying, warning, and addressing concerns as part of their job or as instructed by a registered practitioner, including helping with efficient protection planning.
