Description

• Support patients with severe mental illness through Depot, Clozapine and Wellbeing Clinics.

• Deliver safe, high-quality, person-centred nursing care.

• Conduct mental and physical health assessments, monitoring and treatment under Registered Nurses’ guidance.

• Perform venepuncture, monitor patients’ physical health and arrange medication orders.

• Work collaboratively with pharmacists, nurses and multidisciplinary teams.

• Monitor changes in patients’ conditions and escalate concerns appropriately.

• Maintain accurate patient records and a safe, clean clinic environment.

• Promote patients’ mental, physical and emotional wellbeing.

• Support discharge planning, continuity of care, audits and quality improvement activities.

Requirements

• Foundation degree or Higher National Diploma in Healthcare at Level 5 or equivalent.

• Registration as a Nursing Associate with the NMC.

• Competence in the core clinical skills required for the role.

• Relevant experience in the applied clinical area.

• Experience working with patients and families from diverse cultural backgrounds.

• Ability to work independently and collaboratively within a team.

• Knowledge of the relationship between physical and emotional health.

• Understanding of person-centred care, safeguarding and duty of care.

• Awareness of the impact of mental illness and disability on patients, families and carers.

• Successful Disclosure and Barring Service (DBS) check.
