Main duties of the job
- Are you passionate about making a meaningful difference in the lives of children and young people, as well as their families, living with diabetes?
- We are seeking a compassionate and motivated Paediatric Diabetes Family Support Worker to join our Paediatric Diabetes Multidisciplinary Team, which includes Consultants, Paediatric Diabetes Nurse Specialists, Dietitians, and Clinical Psychologists.
- You will provide personalised support to children and young people with diabetes, and their families, both in hospital and community settings.
- You will play a vital role in promoting well-being, independence, and positive lifestyle choices, while helping families navigate challenges related to health, education, housing, and social care.
- Reports to Lead Paediatric Diabetes Nurse Specialist and Lead Clinical Psychologist.

Qualifications, knowledge, and experience
Essential
- Educated to GCSE (or equivalent) standard, with a pass in English and Maths
- NVQ Level 2 or 3 in Care or equivalent
- Evidence of recent learning or development in health
- Contribute to one’s own personal development
- Previous experience in a clinical environment
- Excellent communication skills with the ability to interact appropriately with patients, relatives, team members and other staff groups
- Ability to work flexibly across core hours during the week and weekends to meet the needs of the service
- Ability to travel between hospital and community bases may be required. The individual may be expected to visit patients at home and in community hubs, as well as between the hospital sites
