The Band 4 Nursing Associate will work as part of the multidisciplinary diabetes admission avoidance team to support adults with diabetes who are at risk of hospital admission. The post holder will deliver high-quality, person-centred care across community and outpatient settings, supporting early intervention, monitoring, education and coordinated care to help prevent unnecessary hospital attendance and admission.

Main duties of the job

The Diabetes admission avoidance team is a new service to identify patients at risk of recurrent hospital admission. The service will focus on rapid clinical assessment, early intervention and prevention of an emergency admission.

The post holder will work as part of a multidisciplinary team delivering safe, effective and compassionate care in line with Trust values and clinical standards. The role requires excellent communication, organisational skills and the ability to work flexibly in a dynamic environment with our most complex patient group. The post holder will contribute to patient assessment, care delivery and service improvement initiatives, ensuring high standards of care and a positive patient experience.

Person Specification

Knowledge and experience

Essential

  • Registered Nurse Associate Training completed

Desirable

  • Demonstrable experience in an acute hospital setting
  • Completed Preceptorship

Qualifications

Essential

  • Having gained a Foundation Nursing Associate degree

Desirable

  • NMC Registered Nursing Associate
  • Having gained a Foundation Nursing Associate degree
  • Basic Computer skills

Aptitude and Abilities

Essential

  • Good communication skills, written and verbal, with proven ability to demonstrate fluency, clarity and effectiveness at all levels.
  • Proven ability to prioritise workload
  • Time management skills

Desirable

  • Proven ability / demonstrable experience of – Directing and instructing others
  • Teaching and mentoring skills
