Job Summary
Are you currently a B3 or B4 looking for part-time opportunities as a B4 community falls support worker?
Do you have a driver’s license and would like to visit patients in their own homes and provide holistic assessment and care to patients who have fallen or are at risk of falls?
Do you have an interest in facilitating exercise classes and providing education to patients?
Main duties of the job
- To respond quickly to all referrals for patients who have fallen or are at risk of falling, register and triage the referral in accordance with the KPI
- To visit patients in their own home or place of residence and complete a comprehensive, evidence-based falls assessment and identify, order any equipment, additional care, or support required to enable the person to remain at home safely.
- To facilitate and support the Luton Falls Team in delivering all aspects of the falls strength and balance classes/ group programme as necessary to include: patient reviews, exercise assessment, education, and psychological support.
- To analyse facts and make judgements within one’s own competency level. This will involve the use of skills to evaluate the patient’s condition and adapt the assessment and recommendations as necessary.
- Based on the falls assessment, make recommendations to patients and their carers that will reduce the risk of further falls.
- To make referrals to other services that will reduce the risk of further falls and promote patients’ independence.
- To follow up with patients to ensure that equipment has been delivered and care is in place.
- To maintain and update patient records promptly using an electronic system and to work closely with the Falls Coordinator to collate and analyse associated data.
- To be responsible for planning and prioritising own workload.

- Experience of working in the community
- Time management, ability to organise and prioritise own work

- Experience in SystmOne or other similar electronic patient records
Knowledge and Skills
Essential
- Car owner and UK driver’s licence
- IT skills, use of Microsoft Word, Excel, and PowerPoint

- Providing education and advice
Qualifications and Training
Essential
- NVQ level 3 or Degree or equivalent, e.g., BSc, MSc, or Diploma entry level qualification into the Physiotherapy profession.
- Completion of the application form legibly and clearly
