We are looking for a highly organised and proactive Care Coordinator to lead and coordinate administrative services within our integrated care community team. This is a pivotal role supporting the delivery of high-quality patient care by ensuring smooth, efficient and professional administrative operations.

Main duties of the job

The Care Coordinator within an integrated community care team will be responsible for high-quality customer service in recording contact information, supporting the initial prioritisation of contacts, subsequent feedback, and ongoing liaison with referrers and relevant others.

The post holder will be the central coordination point for local multiDisciplinary activity, organising ongoing regular meetings to facilitate progress of active cases with relevant team members, including coordinating Multidisciplinary Team meetings to facilitate the provision of care for complex patients/clients.

The postholder will have access to people receiving care/patient facing, and as part of this role a Standard DBS will be required.

Person Specification

Qualifications

Essential

  • Good general education
  • Able to demonstrate a good command of the English language, verbal and written
  • Knowledge of Office Administration/Customer services acquired through training or experience to NVQ level or equivalent qualification.

Knowledge and Expertise

Essential

  • Knowledge of Health and Social Care administration and processes
  • Knowledge of Microsoft Office applications, internet and email

Desirable

  • Knowledge/familiarity with medical terminology

Experience

Essential

  • Able to work well in a team.
  • Experience of working in an Office Environment
  • Experience/knowledge of issues of working with confidential information.
  • Experience of working with healthcare professionals/previous experience in the NHS.

Desirable

  • Working in a busy and demanding environment.
