The Care Co-ordinator is key in primary care as the link between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.

The Care Co-ordinator will be part of North PCN and will be an administrative, non-clinical post, and we welcome applications from anyone with Health or social care expertise.

The Care Co-ordinator is a crucial function within the Team and will serve as the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.

To maintain IT-based information systems and be responsible for the creation of necessary performance statistics.

The Care Co-ordinator will help to reduce inequities in health and social care, especially for people with long-term conditions. The role is based on an idea of encouraging independence and partnership-working.

We are part of Joined Up Care Derbyshire and provide community health services to the patient population of over one million people living in Derbyshire and Derby.

In our community hospitals and more than 30 health centers, clinics, GP practices, schools, care homes and, increasingly, in people’s own homes via virtual consultations, we employ over 4,200 substantive staff who care for patients.
