Job Summary
In partnership with Macmillan Cancer Support, we are recruiting a Macmillan Cancer Social Prescriber (Band 4) to support the delivery of the Improving my Cancer Journey (ICJ) service within the Primary, Community and Intermediate Care (PCIC) Directorate across Cardiff and the Vale.
This role supports people affected by cancer through a non-clinical, community-based social prescribing approach, working primarily within primary care and community settings.
The postholder will act as a friendly point of contact from referral, holding personalised conversations to understand what matters most to people and supporting them to access community, voluntary sector, and wellbeing services alongside their cancer care. Approved ICJ conversation tools will be used in line with agreed protocols to support consistent, high-quality non-clinical discussions.
Working as part of the wider ICJ team and under the supervision of the Macmillan Project Manager, the postholder will work closely with primary care, cancer teams, community organisations, and third sector partners. The role focuses on information, signposting, and navigation, with clear professional boundaries and appropriate escalation when required.
Welsh language skills are desirable; Welsh and/or English speakers are welcome to apply. This post is fixed-term due to Macmillan funding, with potential for extension subject to future funding. Secondment applicants must obtain line manager approval before applying.
Main duties of the job
Act as a member of the Improving my Cancer Journey (ICJ) team within Primary, Community and Intermediate Care (PCIC), supporting people affected by cancer through a non-clinical, person-centred approach.
- Act as a friendly and accessible point of contact for people referred to the service, providing support through face-to-face, telephone, and virtual contact.
- Hold structured, personalised conversations to understand what matters most to people, focusing on non-clinical, practical, emotional, and social needs.
- Use approved ICJ conversation tools in line with agreed protocols to support high-quality non-clinical discussions and appropriate signposting.
- Provide clear information and signposting to the community, voluntary sector, and wellbeing services, supporting individuals to navigate available resources.
- Maintain appropriate follow-up contact to support engagement, recognise changes in circumstances, and escalate concerns when outside the scope of the role.
- Build and maintain effective working relationships with primary care, community and third sector partners to support joined-up, person-centred pathways.
- Accurately record interactions and outcomes using approved digital systems, contributing to service monitoring, evaluation, and improvement.
Person Specification
Qualifications and Knowledge
Essential
- NVQ Level 3 or equivalent experience
- GCSE English Language and Mathematics (A-C) or equivalent
- Evidence of ongoing learning or continuing professional development
- Understanding of person-centred care and holistic, non-clinical support
- Basic understanding of the health and social care environment
- Awareness of safeguarding principles and the importance of confidentiality, data protection, and information governance
Desirable
- Health, social care or community development qualification (NVQ 4, diploma level modules)
- Training or education related to cancer, social prescribing, and or wellbeing support
- Knowledge of NHS Wales cancer policies (e.g., Quality Statement for Cancer, Cancer Delivery Plan)
- Knowledge of Macmillan Cancer Support and the role of voluntary/community organisations
- Familiarity with healthcare terminology
Experience

- Experience in a role involving direct contact with the public, patients, carers or community members
- Experience providing support related to wellbeing, social issues, practical needs or community resources
- Experience working collaboratively with primary care, community groups, voluntary sector or third sector partners
- Experience of multi-agency or multidisciplinary working

- Satisfactory Enhanced DBS clearance, including an Adults Barred List check
