Job summary

Table of Contents

  • To support the integrated 0-19 Healthy Child Service (HCS) in providing an innovative and high-quality service to improve health outcomes for children, young people and their families
  • To support the assessment and review of clinical needs
  • To work without direct supervision to support clients as part of the universal and Targeted aspects of the service provision
  • To liaise with colleagues within the clinical team to assist in the coordination and development of the service.
  • To develop specialist interest areas in order to enrich the knowledge and skills set within the Norfolk Children & Young People’s Health Service.

Main duties of the job

To assist the School Nurses in the management and delivery of care to clients. Duties may include:

Contacts with young people will be delivered in a variety of ways such as families’ homes and venue-based settings, such as schools at a variety of locations across the collaborative localities

Building on existing assessment of families’ needs and delivering evidence-based health advice without direct supervision.

Receiving referrals, triaging and using appropriate signposting.

Working jointly with School Nurse for Specialist families. This may include:

  • Communicating complex information with other agencies such as social care staff regarding work HCS are delivering to the young person.
  • Support to clients with more complex needs.
  • Completing Duty – reviewing waiting lists, triaging tasks, booking in contacts and running reports within System One.
  • Working jointly with outside agencies who may also be supporting a young person.

Person Specification

Qualifications

Essential

  • NVQ level 3 in health, social care, education or an equivalent.

Desirable

  • Foundation degree in relevant area.

Experience

Essential

  • Recent experience of working with children and young people and families.
  • Building professional relationships with parents/carers
  • Demonstrates the ability to safeguard children.

Desirable

  • Experience of working with families in the community
  • Planning and facilitating groups to deliver health promotion messages

Skills

Essential

  • Good communication skills and the ability to change communication style depending on the audience.
  • Ability to analyse information and use reasoning skills to assess clients’ needs.
  • Robust IT skills and clear record keeping.
  • Understanding of the public health agenda and national and local policies relating to Children and Young People’s health and wellbeing, including Working Together to Safeguard Children.
  • Understanding key public health resources to support the children and families of Norfolk.

Desirable

  • Ability to apply and use assessment tools to support a childs developmental assessment and identify need
  • Knowledge of Systm1
  • Experience in or knowledge of child development
