In order to continue developing a customised Community Care/Support Service throughout the Buckinghamshire region, we are looking for an experienced Care Coordinator. For clients to continue receiving care in their homes and in the community, the Care Coordinator organises and arranges for individualised care and resources. Because of the fast-paced nature of the job, someone who can maintain composure under pressure and meet deadlines is needed.
The chosen applicant would work at our Amersham office Monday through Friday from 9 a.m. to 5 p.m., with weekend availability required for on-call coverage. They would also need to be well-versed in the Bucks region.
Care Co-ordinator Jobs UK primary responsibilities and duties include:
- Make sure that all applicable policies, procedures, and regulations are followed when providing care to our clients.
- Take responsibility for the carer roster allocations to guarantee that every client’s demands are satisfied.
- Assess prospective clients to determine their unique care requirements.
- Examining current care plans and reevaluating current clients
- Communicate with all necessary family, medical, professional, and other resources.
- Deliver top-notch customer service to all of the company’s internal and external clients.
- Conduct prompt spot checks, supervisions, and care worker evaluations.
- As a member of a rotating team, provide on-call coverage for after-hours services.
- Ensure that management and employees are communicating clearly and effectively.

- Make sure that you efficiently manage the hours and rotas of your employees. This includes making sure that employees have the proper breaks and rest periods, have allotted travel time, are working the minimum and maximum number of hours per week, and have accurate electronic timesheets.
Post-interview staff recruitment files are current. - Work adaptively to fulfil the home care service’s operating requirements and provide community care when needed.
- If necessary, transfer employees from one service user to another.
