Job Summary
The post holder will work as part of a designated member of the stoma nursing Team delivering evidence based high quality clinical care that focuses on the direct needs of the patient including those individuals with chronic conditions and cancer. The post holder will work under the direction and support of a Registered Nurse by following the Health Board’s protocols, policies, procedures, pathways, clinical governance requirements and care plans.
Main duties of the job
The Assistant Practitioner role will also require the post holder to:
- Ensure effective communication with patients, carers, multidisciplinary team and the wider health and social care community.
- Practice in accordance with Health Board standards and statutory requirements and operate within the boundaries of the role and assessed competencies.
- Use personal effectiveness and knowledge to contribute to the development and delivery of a quality service.
- Work proactively as a member of the multi disciplinary and multi professional healthcare team, in support of registered practitioners.
- Manage a small defined caseload of patients under the supervision of the registered nurse.
- Undertake the care of stoma/wounds that require simple or more advanced dressings, document care given and report back findings to relevant nurse.
- Have a remit, assessing patients for their stoma needs involving physical and psychological observations within the acute and community settings. Assisting patients to achieve optimum physical and emotional wellbeing and comfort.
- Discuss and make suggestions for adjustments to individual care plans and their health and well being needs with the care team and understand own role in delivering care to meet those needs.
- Home Visits to review patients maybe needed to be undertaken in this role.

Qualifications and/or Knowledge
Essential
- Level 4 vocational qualification in a relevant health care associated subject / Certificate in Higher Education Health Care Support Worker
- GCSE Maths and English Grade C or above or equivalent level 2 or above essential skills qualifications
- Evidence of continuing personal development relevant to the role in health care
- Able to demonstrate understanding of the HCSW Code of Conduct, All Wales Delegation guidelines and all Wales Band 4 Assistant Practitioner Governance Framework.
Desirable
- Local knowledge of statutory and voluntary services
- Teaching or assessment qualification
Experience
Essential
- Significant and recent experience of providing relevant clinical care
- Interpersonal and communication skills and the ability to manage/diffuse challenging or threatening situations
- Experienced in undertaking a range of clinical skills relevant to the post as per role scope of practice and the national governance framework e.g. oCatheter care (including removal) oMonitor and record vital observation and interpret results oReviewing risk assessments oMonitor and interpret blood glucose. oExperience of supporting an RN with wound care
Desirable
- Experience in a range of clinical skills including palliative care
- For community roles: experience of providing care in a community or primary care setting.
- Administration of medicines via oral, topical, SC, IM routes
