The Royal Free Community Neurological Conditions Management Team (CNCMT) at the Neurological Rehabilitation Centre in Edgware offers a multidisciplinary approach, collaborating with a consultant neurologist and other healthcare professionals.

The role involves both face-to-face and virtual working, with some travel across North Central London (NCL). You will manage a caseload independently, guided by the qualified therapists, to deliver holistic, patient-centred care. Key responsibilities include patient education, exercise programmes, promoting self-management, working independently under guidance from qualified therapists at clients’ homes or in outpatient settings, and organising and running group treatment sessions. You will also liaise with the inpatient team to support patient flow out of rehab, contribute to service development, and represent the team in meetings and training sessions. Strong communication skills and a commitment to delivering world-class care are essential.

Main duties of the job

  • To support the Community Neurological Conditions Management Team (CNCMT) multidisciplinary team (MDT) to deliver rehabilitation and condition management services to patients with neurological conditions within the outpatient setting, the home environment, and the local community.
  • To provide goal-directed clinical interventions to patients with neurological disability who have been previously assessed by a member of the MDT, and to progress this treatment with indirect supervision of the lead therapist
  • To progress treatment without the direct supervision of a therapist, but under the direction of the relevant qualified therapists.
  • Lead the delivery of therapist-prescribed group exercise and education programmes
  • To record and report back in the relevant records in line with national and local standards.
  • To undertake routine maintenance checks of clinical areas and equipment within designated clinical areas.
  • To participate in 7-day work as required by the site
  • To regularly carry out administration tasks to support patient care
  • To work flexibly to enable the provision of services that meet patient needs.
  • To support patient allocation with rehabilitation assistant colleagues
  • To carry out patient exercise programmes as prescribed by the therapists in either patient homes, community settings (i.e., gym group), or outpatients as required

Person Specification

Experience

Essential

  • Experience working at a Band 3 level in a physical rehabilitation setting
  • Experience working within a therapy setting, delivering care to adults in an acute and/or community NHS setting
  • Knowledge and experience of working with common neurological conditions
  • Knowledge of the ranges of equipment relevant to the clinical area

Desirable

  • Knowledge of differing models of practice or approaches to treatment

Education & Professional Qualifications

Essential

  • Relevant qualification e.g., C & G Health & Social Care NVQ 3 or equivalent

Desirable

  • Training courses relevant to the role
  • GCSE English and Maths or equivalent
