Main duties of the job

This post will be part of a team who care for children and young people with Diabetes. The post holder will work closely with hospital, community and primary care services.

The key purpose of the role is to deliver high-quality targeted family support to families of children diagnosed with diabetes. The post holder’s responsibilities will include: supporting families with adjustment to diagnosis, providing parenting support, supporting clinics, and forging working relationships with local schools and services. They will also be responsible for delivering group interventions for parents and young people.

The support will be provided in the home, community and hospital settings. The post holder will work collaboratively with the family, diabetes team and any professional or voluntary services supporting the family and will be required to develop their knowledge about diabetes and how it is managed in children, and in keeping with treatment and technology advances.

The post holder will act as a role model in the delivery of patient care and will provide support to the team in the effective management of the service.

The post holder will provide effective liaison with the multi-disciplinary team and other professional colleagues and agencies involved within the child’s care.

The post holder will be required to act in a manner consistent with the NMC Code of Professional Conduct, carrying out their duties in accordance with Trust policies, guidelines and procedures.

Person Specification

Core role requirements

Essential

  • Registered Nurse Associate qualification.
  • Active NMC registration.
  • Previous experience working with children and young people in a clinical setting.
  • Diabetes knowledge and experience.
  • Understanding of safeguarding, child-centred care, and transition to adult services.
  • Good IT skills, including use of electronic patient records and digital diabetes platforms.

Desirable

  • Awareness of currently available diabetes technology.
  • Awereness of BPT and NPDA.
  • Experience supporting patients using insulin pumps and continuous glucose monitoring (CGM) systems.
  • Experience of participating in quality improvement or service development projects.

  • To be able to perform under pressure

  • To be flexible, with capacity to adapt to new workload demands
  • To be able to manage own caseload
  • Excellent communication and interpersonal skills with children, young people, and families.

Desirable

  • Experience in an outpatient, community, or specialist paediatric setting.
  • Experience contributing to education programmes for children, young people, and families.

  • Possession of a valid UK driver’s licence and access to a vehicle
