An exciting post is now available for a Band 4 Therapy Assistant to work within multidisciplinary community settings and support the delivery of high-quality therapy interventions to patients within their own homes.
This post will be essential in order to support the ongoing care and management of clients within the community who are in receipt of the Integrated Placement Of Care Hub (IPOCH) General Nursing Assistant (GNA) package of care.
This role offers the opportunity to enhance skills within the community therapy services, linking in with the wider support network and being part of an integrated support system. You will work across our Discharge to Assess beds and across all care communities as required.
It is expected that the location of this post will be within the IPOCH Transfer of Care Hub, but with an assumption of supporting our colleagues across the CCICP footprint when needed.
The successful candidate will have an Assistant Practitioner qualification (ST0215 Healthcare Assistant Practitioner Level 5)or equivalent.
Main duties of the job
- Our Band 4 Therapy Assistant will take responsibility for the management of a defined workload of patients who are currently receiving a care package from the GNA service, or reablement/block care provider, following triage by the qualified therapist.
- The service focuses on a ‘home first’ approach; one of rehabilitation and promotion of independence for patients leaving acute/bed-based settings or requiring step-up care in their own homes. It also supports end-of-life patients in their own homes, providing a dignified and quality approach to care for the patients and their families at a challenging time.
- The role will include independent assessment & formulation and progression of a person-centred rehabilitation plan. To work as part of the team providing a comprehensive rehabilitation service under the guidance of qualified staff for patients in their care.
- Good communication between the patient, families, IPOCH, and therapy teams will be essential- providing realistic expectations of the service and providing up-to-date information on the current presentation of every patient on the caseload.
The successful candidate will undertake record keeping using an electronic device for which training will be provided.
