As part of community transformation, an exciting role has been developed between TEWV & Tees Valley Primary Care networks (PCN’S).

We are looking for an enthusiastic Community Associate Practitioner to work within the Tees Valley.

The role will be split 50% within Primary Care Networks across the Tees Valley and 50% within the TEWV mental health community hubs.

The role is to ensure that the physical health needs of patients on the SMI register within GP surgeries are met. The practitioner will be expected to link in with individual GP practices to identify patients who are outstanding for physical health checks.

An assertive outreach approach will be adopted to engage patients on the SMI register to have their physical health investigations carried out. A kit will be provided to ensure that physical investigations can be carried out, at times, outside of a clinical environment. Full training on the kits will be provided.

  • The successful candidate will be able to work as part of an integrated Community Mental Health Team and Primary Care Services. The role will involve working alongside the Leadership Team and Care Co-ordinators towards providing recovery-focused interventions with service users and support carers.
  • Managerial and clinical supervision will be provided to support the successful candidate in managing their caseload.
  • There are varied opportunities for training and development, with clinical development needs being identified and monitored through appraisal.

The successful individual will need a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent to be recruited into this position on Band 4.

Qualifications

Essential

  • Foundation Degree in Health and Social Care
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)

Experience

Essential

  • Previous experience of working with people with mental/social health needs or learning disabilities.
  • Previous experience as a Band 2/3 HCA or equivalent in a care environment

Knowledge

Essential

  • Person-centred care
  • Willing to undertake learning in areas of relevance to the role
  • Evidence-based knowledge of current practices in the field
  • Awareness of legal frameworks, including Mental Health legislation
  • Care Programme Approach
  • Knowledge of Microsoft Office applications (within agreed timescale)

Skills

Essential

  • Effective verbal and written communication skills
  • Time management and ability to prioritise effectively
  • Competency in record keeping (within agreed timescales)
  • Breakaway techniques (within agreed timescale)
