Job Summary

Active Programmes is a well-developed multi-disciplinary team of nurses, physiotherapists, and specialist Therapy assistants, all of whom cover both the Pulmonary and Cardiac rehabilitation. The team also comprises a small Cardiac & Respiratory Mental Health Team consisting of nurses and occupational therapists.

Main duties of the job

  • To be legally responsible and accountable for all aspects of your own work, including the management of patients in your care, ensuring a high standard of clinical care for the patients under your management.
  • To gain valid informed consent and have the ability to work within a legal framework with patients who lack the capacity to consent to treatment.
  • To deliver rehabilitation programmes in groups with qualified support, and in patients’ home settings, independently and according to local protocols and guidelines.
  • To be able to take an accurate and reliable set of baseline observations, including blood pressure, oxygen saturations, pulse, and to be able to identify when they are abnormal and what action is appropriate.
  • To frequently assess patients according to defined protocols requiring prolonged concentration to investigate and analyse the assessment to formulate individualised management and treatment plans.
  • To use continuous assessment to evaluate treatment outcomes and adjust treatment plans accordingly, appropriately referring to other agencies to meet patients’ needs and promote integration of services.
  • To be able to facilitate patients in setting SMART goals and to review them when necessary.

Person Specification

Education and Qualifications

Essential

  • Level 4 Diploma in a relevant discipline, such as a Personal Trainer/fitness instructor. Or Level III NVQ in a relevant topic i.e., Health & Social Care or equivalent, with experience of working in an exercise delivery environment.
  • Maths & English GCSE grade C/4 or above OR core literacy and numeracy skills.
  • Able to demonstrate a commitment to personal development.

Desirable

  • IT training/qualification to ECDL level or equivalent
  • Later Life Training qualification or equivalent.
  • BACPR Phase IV qualification

Experience

Essential

  • Relevant experience working as a Therapy assistant with relevant experience at the Band 3 level.
  • To have worked with patients who have a range of mobility, balance, and long-term conditions.
  • To have managed own clinical caseload and worked independently of a qualified therapist
  • Experience in liaising with other agencies, services, and professional colleagues.
  • Experience as part of a multidisciplinary team.
  • Knowledge of the roles of all members of the interdisciplinary team.
  • To have knowledge about functional ability regarding personal and domestic activities of daily living.
  • Knowledge of the duty of, and the STH’s policy regarding confidentiality.

Desirable

  • To have experience of working in Cardiac &/or Pulmonary Rehabilitation
  • Experience in supervising staff
  • Experience of working in the community.
  • To have knowledge of recent health and social care polices relating to the care of older people.
  • Experience in coaching/ mentoring/training other staff.

Skills and Knowledge

Essential

  • Leadership and motivational skills, and the ability to meet deadlines and multitask.
  • Be computer literate.
  • Awareness and experience of using reflective practice.
  • Moving and handling skills.
  • A car driver with a valid UK driving licence.

Personal Qualities

Essential

  • Innovative and flexible approach.
  • Awareness of own limitations within the scope of the role.
  • Able to meet the health requirements of the role and the Trust.
  • Awareness of cultural diversity.
  • To be motivated, enthusiastic and confident.
