Job summary
This is an exciting opportunity to join an established and experienced team who will support you to define the principles of the New Neighbourhood Mental Health Centre. You will be encouraged to effect real positive change for the community, working alongside the people in the community and local voluntary organisations to inform future potential Neighbourhood Mental Health Centres in the city.
Main duties of the job
Acting as a Named Worker for a designated caseload to ensure service users receive a consistent approach and a named person to contact. This will involve building a working relationship and promoting an individual’s ability to manage their own needs and independence. As a named worker, you will work in collaboration with the Multidisciplinary team to support the overall care and treatment plan of the individual.
A named worker for a Senior STR Worker would mean engaging with a service user to offer care and treatment where care is stable. If any changes occur in relation to deterioration in mental health or physical health, the MDT will consider more appropriate options for that service-user in relation to the care plan/DRAM.
A Senior STR Worker acting as a named worker will update a care plan and DRAM and change the content with support from the MDT, who would guide principles of this change.
Contribute to the collaborative planning and review of Recovery Goals with service users and team members.
To have a working knowledge of care and support planning.
Support the completion of patient-reported outcome measures (PROMs) as part of supporting care planning and reviews of care.
To be aware of and abide by service policies re confidentiality and information sharing.

- Access to a vehicle and hold a UK driving license
- Evidence of flexible working and managing own diary
- Experience of talking to service users and managing difficult conversations
- Knowledge of benefit system and current socio-political climate
- Experience of liaising and signposting to other services
