Overview

We are seeking compassionate and dedicated Community Care and Support Workers to provide high-quality assistance to individuals within their own homes and care settings. Duties

• Assist clients with daily living activities such as personal care, meal preparation, and medication management
• Support clients in maintaining their independence by helping with mobility and household tasks
• Develop and follow personalised care plans tailored to individual needs
• Provide companionship and emotional support to clients, fostering a positive environment
• Maintain accurate records of care provided and report any concerns or changes in client condition
• Utilise IT systems to update care documentation and communicate effectively with team members
• Drive clients to appointments or social activities as required, ensuring safe transportation
• Uphold health and safety standards, including infection control procedures and safeguarding protocols

Requirements

• Valid driving licence and access to a reliable vehicle are preferred
• Experience working within a care home or providing home care is advantageous but not essential; training will be provided
• Ability to communicate effectively in English, both verbally and in writing
• Strong IT skills for record-keeping and communication purposes
• Excellent communication skills with the ability to build rapport with clients from diverse backgrounds
• Compassionate nature with a genuine desire to support vulnerable individuals
• Flexibility to work various shifts, including evenings or weekends if required
• Ability to work independently whilst following care plans accurately
