Job Summary
The post holder will be part of a team acting as a named point of contact for families facing the loss of a child or young person.
To provide a coordinating role in ensuring that when a baby, child, or young person dies (0-18 years), statutory responsibilities in accordance with ‘Working Together to
Safeguard Children 2024 and ‘Child Death Review Statutory Guidance’ 2018 are met, and that families/carers are effectively supported and signposted.
Be a key member of the Calderdale and Huddersfield Foundation Trust (CHFT) Sudden Unexpected Death in Childhood (SUDIC) team and the CHFT end-of-life care team.
The post holder will support the designated doctor for child deaths and act as a liaison between relevant partner agencies.
Main duties of the job
- Promote processes for the management of child death in line with ‘Child Death Review Statutory Guidance’ 2018, to ensure there are robust systems in place for responding to child death across CHFT
- To work in close partnership with the CHFT SUDIC Team and the End of Life care Team. The postholder would work closely with the Safeguarding team when there are safeguarding concerns in relation to the nature of the death
- To ensure that in all cases a key worker has been assigned to each family, either through the Joint Agency Response process (unexpected deaths) or Early Response (predicted deaths) in consultation with the relevant CDOP.
- Act as a point of contact within CHFT for the management of information regarding baby, children, and young people’s deaths.

- Current NMC registration RN Child /RSCN
- First-level degree
- Experience of working with and supporting bereaved individuals and families
- Proven experience working with children and families with complex medical/ social needs/distressing circumstances
Desirable
- Willing to undergo training as necessary e.g., Child Death Review/Child Bereavement Trust
- Experience of the management/child death process
- Post-registration teaching/mentorship qualification
- Experience of working without direct supervision, but as part of a team to meet deadlines

- Postgraduate qualification in public health nursing e.g., health visiting or school nursing
