Do you have experience working in the community as an HCA, a reablement support worker, a rehabilitation support worker, a therapy assistant, Therapy Technician?
Do you enjoy working in the Luton community, providing a prevention and rapid intervention service for people who have fallen or are at risk of falls?
The service works shifts from 8pm until 6pm, 7 days per week, with weekend work on a rota basis. A typical shift pattern for a full-time member of staff may include 4 working days over a 7-day period with 3 rest days.
Due to the nature of the role, it is essential that applicants can travel effectively and independently daily to and from the base, clients’ homes etc whilst transporting equipment. If necessary, adjustments can be considered in accordance with the Disability Discrimination Act.
Main duties of the job
We have an exciting opportunity for motivated, well-organised, and enthusiastic individuals to join our falls team as a Community Falls Support worker. You will be based in Luton as part of a small proactive team, but will be able to work autonomously using your own initiative, have good communication skills, and be IT literate. We offer the successful applicants a variety of interesting work in a friendly and supportive team. Training will be provided, and the Trust is committed to staff development.
To respond quickly to all referrals for patients who have fallen or are at risk of falling, arranging to visit them as soon as possible.
To visit patients in their own home or place of residence.
To complete a comprehensive, evidence-based falls assessment and identify any equipment, additional care or support required to enable the person to remain at home safely.
To support the Luton Falls Team in delivering all aspects of the falls strength and balance classes/ group programme as necessary to include: patient reviews, exercise assessment, education, and psychological support.

- Effective communication skills, written, verbal, and report writing skills
- Time management, ability to organise and prioritise own work
- Car owner and UK licence driver
- Able to use IT, MS Word, and Excel
Desirable
- Detailed understanding of a wide range of medical conditions.
- Experience in using Systm 1
Experience

- Experience of working with OT/PT as B3/B4
- Experience in facilitating strength and balance groups
- Experience working in the community

- Equipment ordering, i.e., Transfers and Mobility
- Working in Falls-related cases
Education and Training
Essential
- NVQ level 3 or Degree or equivalent, e.g. BSc,MSc or Diploma entry level qualification into the Physiotherapy profession.

- Training related to Falls prevention
- Training related to Strength and balance exercises
