The purpose of the Breast Cancer Patient pathway navigator, supporting SFU, is to support the Personalised Stratified Follow-Up (PSFU) Clinical Nurse Specialists (CNSs) to deliver safe, effective follow-up care to patients who have had treatment for breast cancer. The role is responsible for overseeing the transfer of patients from the Somerset Cancer Registry database (SCR) to the Somerset Remote Monitoring System (RMS).

The role is responsible for functions within and related to the RMS, including the escalation of variances, generating letters, and making appointments. They will support the provision of Personalised Cancer Care to patients on the PSFU pathway in conjunction with the Breast PSFU Clinical Nurse Specialists. They will coordinate the surveillance programme and ensure that relevant letters are sent out to the patients and their GP. They also ensure that due tests are escalated to the CNSs. They will assist with collecting and collating data about the service to assist with monitoring and improving it and ensuring patients benefit from Supported Self-Management interventions.

Main duties of the job

The right candidate for this role will be a keen and experienced administrator within the NHS with an ambition to learn clinical administration to support the nursing function withing long term Breast Cancer service.

A key aspect of the role involves daily and direct (e.g., face-to-face and telephone) communication with breast cancer patients and other health and social care professionals.

The role includes undertaking patient-facing duties, including aspects of Holistic Needs Assessments and End of Treatment Summaries in conjunction with the Breast PSFU CNSs. The information and nature of the communication required are sensitive due to the nature of cancer. Communication in this context requires a high degree of empathy, understanding, diplomacy, honesty, and integrity.

Education and professional qualifications

Essential

  • RSA 2 or RSA 3 word processing/typing or equivalent experience
  • Knowledge of medical terminology
  • English and Maths at GCSE level or equivalent

Desirable

  • A diploma-level qualification or equivalent experience
  • Courses / further study attended to demonstrate evidence of personal development

Experience

Essential

  • Previous experience of working with the public and/or patients in a healthcare environment
  • Experience of interaction with all levels of staff
  • Previous administrative and organisational experience
  • Experience in dealing with non-routine and complex problems

Desirable

  • Experience of managing and prioritising own workload and working to deadlines
